The link for the form to change to a Spanish licence is:-

The fee is approx 22 euros. no charge for over 70.

You will need:-
Passport, DNI or residencia
Original driving licence
Colour Photo 32 x 26 - white background & head filling the photo
Official medical certificate
Possibly a Certificado de Empadronamiento
